<br />Diesel Spill Chronolgy <br />Southdown, Inc. <br />Lyons Cement Plant <br /> <br />4/15/96 Quarry supervisor noted diesel oil floating on <br />stormwater inside containment dike and notifies <br />facility compliance manager. <br />4/15/96 Southdown's facility compliance manager notifies <br />Julie Looter of Colorado State Department of Labor <br />and Employment (responsible for Aboveground Storage <br />Tanks (AST's)) by telephone of fuel oil spill/leak. <br />Informed by Ms. Looter that cement plant is exempt <br />due to exemption for mine sites. <br />4/16/96 Southdown requested ERM Rocky Mountain <br />(environmental consulting engineers) to complete a <br />comprehensive site assessment on an expedited <br />basis. <br />4/26/96 Confirmed telephone contact with Ms. Looter by <br />letter. <br />5/20/96 Southdown notifies Susan Jones-Hard of CO <br />Department of Public Health and Environment by <br />telephone of diesel issue. <br />5/22/96 CO Department of Public Health and Environment <br />notified by letter. <br />6/05/96 Notified State Department of Minerals and Geology <br />of situation. <br />6/17/96 ERM Rocky Mountain completes "Subsurface Invest- <br />tigation". <br />7/02/96 ERM Rocky Mountain completes "Design Document" <br />7/12/96 Southdown approves "Remediation Plan" for <br />installation by ERM Rocky Mountain. <br />7/12/96 Southdown approves tank farm renovation to prevent <br />future issues. <br />9/96 Remediation system expected to be in operation and <br />tank farm renovation expected to be completed. <br />
