Rased upon our records, the anniversary date of your mining operation peimit is
' October 26 and annual fees for the year(s) 1982 _, totalling
S 350.00 are due on that date. Fees are based upon a schedule of
5350.00 if the operation is an open mining operation of 10 acres ~~r more and
j275.00 _for any other operation IO acres or more.
• Also, to bring the Division up to date as to the mining and reclaration on the
site, please submit a report and map (to a scale cf not Less than 100' to the
' inch and not more than 660' to the inch), indicating the phases ci' th2 reclama-
i ti on plan which have been completed, correlated wit'1 the timetablE•. The report
and map shall contain the following information:
~ --
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed for that year,
3) The location and number of acres in the various stages of reclamation
(backfiZled, graded, topsoil replaced, re vegetate d),
4) The species of vegetation planted, the locations and approximate dates
of planting,
5) The type and approximate quantity of fertilizers, organic material or
soil conditioners used and location of use,
6) An appraisal of the success of reclamation efforts, and in the event
of failure of any efforts, a short narrative en the suspected or determined
causes of such failure,
7) Estimates of the location and number of acres to be affectad in the
coming year, and
8) The effective date of the information provided above shall be no more
than 45 days prior to the anniversary date.
