Two versions of the map are enclosed for your convenience. The original <br />Annual Fee and Report Request Form sent annually by the DMG was not received by <br />Front Range Aggregates. However, another form was requested from your office, and <br />the annual fee and the signed Annual Report Form was sent to the DMG under separate <br />cover. <br />As you are aware, Front Range Aggregates took over the property from Colorado <br />Materials during 2004. Front Range Aggregates occupied the site and mined in the <br />existing pit, as shown on the attached map, for a short period in sate 2004. <br />As the water monitoring sites were dry in late 2004, no water samples were collected in <br />2004. <br />If you have any questions, please contact me, <br />Sincerely, ~~"- <br />li`~ <br />W.
