As of the date of this letter the Division has not received your annual fee and report for <br />January 13, 1996. Therefore, you are in potential violation of the above cited statute and the provisions of <br />Permit No. M-88-087. <br />• If you fail to submit your annual fee and report to our office by June 24, 1996, this matter will be heard at <br />a Formal Public Hearing before the Mined Land Reclamation Board for consideration of possible permit <br />revocation, assessment of civil penalties equal to the annual fee, and forfeiture of your Corporate Surety <br />No. 5552891, in the amount of $5,000.00, at the June 2b and 27, 1996 Board Meeting. If you submit the <br />past due annual fee and report by June 24, 1996, we will withdraw this matter from further consideration. <br />IN ORDER TO AVOID PERMIT REVOCATION, AND POTENTIAL CIVIL PENALTIES, PLEASE SUBMIT <br />$225.00 FOR THE ANNUAL FEE AND YOUR ANNUAL REPORT BY JUNE 24, 1996. <br />The Board Meeting will be held on June 26 and 27, 1996, in Room 318 of 1313 Sherman Street, Denver, <br />Colorado, beginning at 9:00 a.m. or as soon thereaker as the matter can be considered, and you will have <br />the opportunity to be represented and heard. <br />If you have any questions, please contact H.
