BANTA, Dlreetor <br />April 25, 1988 <br />Bettger Sand and Gravel <br />Box 771073 <br />Steamboat Springs, CO 80477 <br />112 Annual Fee Letter <br />In order to maintain your mining operation in compliance with the law, the annual <br />fees, report, and map required by C.R.S. 34-32-116(1)(a) must be submitted <br />annually on or before your permit anniversary date. Fees are based upon a <br />schedule of $350.00 if the operation is an open mining operation and $275.00 for <br />any other operation. Division records indicate you owe $350,00• <br />Permit No,: M-76-051 <br />Operation Name: Bettger Gravel Pit <br />Anniversary Date: 05/08/88 <br />Annual Fees for the Year(s): 1988 <br />Total: $350 (Due on the Anniversary Date) <br />If your records indicate different facts, please inform this office as soon as <br />possible, otherwise, we will expect to receive the annual fees as required by the <br />statute on or before your anniversary date. Also, please include the required <br />Notice of Intent to Continue Mining Operations (Annual Report) to bring the <br />Division up-to-date as to the progress of mining and reclamation on the site. If <br />you cannot include these materials with the fees, please incidate on the enclosed <br />sheet how soon we should expect to receive the material, <br />Submittal of a report and map (to a scale of not less than 100' to the inch and <br />not more than 660' to the inch), indicating the phases of the reclamation plan <br />which have been completed and correlated with the timetable is required.
