Box 467 <br />Golden, CO 80402 <br />RE: Keenesburg Ash Disposal Facility, 2004 Annual Inspection <br />Weld County, Colorado <br />Dear Mr. MacDonald, <br />OF'C~Lp <br />yT p <br />0 <br />~O~ <br />* X876 ~ <br />Colorado Departrnent <br />of PublicHealth <br />and Environment <br />On Thursday March 4, 2004 a representative of the Solid Waste Unit of the Hazazdous Materials and <br />Waste Management Division (Division) inspected the Keenesburg Ash Disposal Facility, located about <br />seven miles northwest of Keenesburg, in Weld County. The purpose of the inspection was to assess the <br />compliance of the facility with the minimum standards of the Solid Waste Disposal Sites and Facilities <br />Act, Title 30, Article 20, Part 1, C.R.S. as amended, and the Regulations promulgated thereunder, 6 <br />CCR 1007-2 (Regulations). A copy of the inspection report is enclosed for your reference. <br />The Boazd of County Commissioners of Weld County granted approval of a certificate of designation <br />for the ash disposal site on April 15, 1987. The Weld County Commissioners approved a Site Specific <br />Development Plan and Special Review Permit #1160 (Second Amended Specia] Review Permit #386) to <br />allow the disposal of mine waste rock at the facility on January 7, 1997. <br />After arriving at the site at 10:00 a.m., Cindi Etcheverry (Weld County Department of Public Health and <br />Environment) and I were greeted by Don MacDonald, at which time I presented my credentials. Byron <br />Walker, with the State of Colorado Division of Minerals and Geology was also present. We first <br />inspected the operating records, which were found to be well organized and comprehensive. <br />We then accompanied Don MacDonald to observe the facility operations. We left the site office at <br />11:30 a.m. <br />
