t <br />QUARTERLY COAL WASTE BANK INSPECTION REPORT <br />CDMR Rules 4.09.1(11) and 4.10.2(2) <br />Mine: Southfield D <br />Waste Bank MSHA ID. <br />Area <br />Inspection Period: First Quarter 2005 <br />Inspection Date: April 4, 2005 <br />General Description or Reference to Site Plan: <br />The refuse area lies west of the main facilities area. Refuse has been placed in designed lifts <br />on a relatively flat terrain. Upslope runoff is diverted away from the disposal area, while runoff <br />from the disturbed area is channeled to a sediment control pond. The mine is in final <br />reclamation and all areas have been sloped to final grade, covered with topsoil, and seeded <br />and mulched. <br />Activity During Inspection: <br />The area was idle at time of inspection. Soil had good surface moisture content. Early <br />season grasses beginning to appear with good density observed in most areas. No signs <br />of erosion or cracks, slumps, or other signs of potential failure were noted on any of the <br />side slopes. Perimeter ditches were open and functional. <br />Drainage: (Discussion of springs. seeps, ovedand and channel flow, underdrain construction and/or <br />discharge, integrity of 100-year 24-hour drainage structures, and evidence of positive drainage). <br />No springs or seeps were observed on the refuse surface. Positive drainage has been maintained <br />on the reclaimed areas. Drainage and diversion structures were intact. All required underdrain <br />construction has been completed. <br />Observation of Fill Construction: (Description of material, compaction, lift thicknesses, slopes, <br />and construction methods). <br />No refuse placement has occurced since December of 2000. Prior to cessation of mining, waste <br />material was placed in alternating lifts of fine and coarse material in acconlance with the plan as <br />described in the,TercaMatrix report dated December 5, 1996. <br />9' <br />)< <br />
