(1989 Supp.) and Permit No. M-80-041 require <br />you to submit a 550.00 annual fee and an annual report on or before April 3 of <br />each year, for the previous years operations, until your rec ama ion bond is <br />released in its entirety, As of the date of this letter we have not received your <br />annual fee and report. Therefore, you are in violation of the above cited statute <br />and the provisions of your Permit No. M-SO-041. Purwant to Section 34-32-124(7), <br />• C.R.S. (1984) you are hereby assessed a civil penalty in the amount of 550.00. <br />If you fail to submit your annual fee, annual report and the civil penalty assessed <br />to our office by June 3, 1991, this matter will be scheduled for Formal Public <br />Hearing before the Mined Land Reclamation Board for consideration of permit <br />revocation and forfeiture of your 52,500.00 Letter of Credit No, TR-3-80, issued <br />March 28, 1980, at the June 26-27, 1991 Board Meeting. <br />IN ORDER TO AYOID PERMIT REVOCATION.
