Dept. of Natural Resources Based upon our records, the anniversaru date of your mining operation permit is October 27 and annual fees for the year(s) 1982 totalling S 350.00 are due on that date. Fees are based upon a schedule of 5350.00 if the operation is an open mining operation of 10 acres or more and 5275.00 for any other operation IO acres or more. . Also, to bring t5e Division uo to date as to the mining and reclamation on the site, please submit a report and map (to a scale of not less than 100' to the inch and not more than 660' to the inch.), indicating the phases of the reclama- tion plan which have been completed, correlated with the ti,retable. The report and map shall contain the following information: 1) Location and number of acres affected during that year, 2) Locations and number o° acres reclaimed for that year, 3) The Iocation and number of acres in the various stages of reclamation (backfilled, graded, topsoil replaced, revegetated), 4) The species of vegetation planted, the locations and aoproxi.rate dates of planting, 5) The type and approximate quantity of fertilizers, organic material or soil conditioners used and location of use, 6) An aporaisal of the success of reclamation efforts, and in the event of failure of any efforts, a short narrative on the suspected or determined causes of such failure, 7) Estimates of the location and number of acres to be affected in the coming year, and 8) The effectioe date of the information provided above shall be no more than 45 days prior to the anniversary date.
