110(2) ANNUAL FEE LETTER PLEASE READ CAREFULLY
In order to maintain your mining operation in compliance with the law, the annual
fees , report, and map required by C.R.S. 34-32-116(3) and 34-32-127(2)(a)(II)(A-D)
must be submitted annually ON OR BEFORE your permit anniversary date. The annual
fee (this is NOT a renewal fee) , report, and map are for the 1990 mining and
reclamation season, and and are due and payable , unless your reclamation bond has
been released in its entirety, before this anniversary date. Operators having
permits under C.R.S. 34-32-110(2) shall submit, in addition to the plan and map, an
annual fee each year.
Division records indicate you owe $120.00.

PENALTIES IN THE AMOUNT EQUAL TO YOUR ANNUAL FEE AMOUNT OF $120.00 WILL BE
AUTOMATICALLY ASSESSED.

already forwarded your remittance, report, and map to us please inform this office
