In order to continue your mining operation, annual fees, report, and m~ip are
required pursuant to 34-32-116(1)(a) of the Colorado Mined Land Reclam;ition
Act, C.R.S 1913, as amended. Fees are based upon a schedule of $350.00 if the
operation is an open mining operation of 10 acres or more and $275.00 ~or any
other operation 10 acres or more.

• If your records indicate different facts, please inform this office as soon as
possible, otherwise, we will expect to receive the annual fees as required by
the statute on or before your anniversary date. Also, piease include the
required Notice of Intent to Continue (dining Operations (Annual Report) to
bring the Division up-to-date as to the progress of mining and reclamation on
the site. If you cannot include these materials with the fees, please
incidate on the attached sheet how soon we should expect to receive ttie
material.
Submittal of a report and map (to a scale of not less than 100' to the: inch
and not more than 660' to the inch), indicating the phases of the reclamation
plan which have been completed and correlated with the timetable is required.
The report and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed for that year,
3) The location and number of acres in the various stages of
reclamation (backfilled, graded, topsoil replaced, re vegetated),
4) The species of vegetation planted, the locations and approximate
dates of planting,
