In order to continue your mining operation, annual fees, report, and map are
required pursuant to 34-32-1]b(1)(a) of the Colorado Mined Land Reclamation
Act, C.i.S 1973, as amended. Fees are based upon a schedule of $350.00 if the
operatt~n is an open ~nininy operation of 10 acres or more and $275.00 for any
other operation 10 acres or mare.

possibla, otherwise, we will expect to receive the annual fees as required by
the st•^tute on or oefore your anniversary date. Also, please include the
required Notice of Intent to Continue ltiining Operations (Annual Report) to
oriny ~he Division up-to-date as to the progress of mining and reclamation on
the site. If you cannot include these materials with the fees, please
incida~e on the attached sheet how soon we should expect to receive the
materiel.
Submit~al of a report and map (to a scale of not less than 100' to the inch
and no more than 560' to the inch), indicating the phases of the reclamation
plan wilicn have been completed and correlated with the timetable is required.
The report and map shall contain the following information:
) Location and number of acres affected during that year,
Locations and number of acres reclaimed fur that year,

;) The location and number of acres in the various stages of
reclam',tion (backfilled, graded, topsoil replaced, revegetated),
;) The species of vegetation planted, the locations and approximate
