In order to maintain your mining operation in compliance with the law, the
annual fees, report, and map requireA by C.R.S. 34-32-116(1)(a) must be
submitted annually on or before your permit anniversary date. Fees are based
upon a schedule of E350.00 if the operation is an open mining operation and
E275.00 for any other operation. Division records indicate you owe E350.

If your records indicate different facts, please inform this office as soon as
possible, otherwise, we will expect to receive the annual fees as required by
the statute on or before your anniversary date. Also, please include the
required Notice of Intent to Continue Mining Operations (Annual Report) to
bring the Division up-to-date as to the progress of mining and reclamation on
the site. If you cannot include these materials with the fees, please
incidate on the enclosed sheet how soon we should expect to receive the
material.
Submittal of a report and map (to a scale of not less than 100' to the inch
and not more than 660' to the inch), indicating the phases of the reclamation
plan which have been completed and correlated with the timetable is required.
The report and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed for that year,
3) The location and number of acres in the various staves of
reclamation (backfilled, graded, topsoil replaced, revegetated), _
4) The species of vegetation planted, the locations and approximate
dates of planting,
