ORCHARD VALLEY MINE <br />QUARTERLY WASTE DISPOSAL AREA INSPECTION REPORT <br />WEST RIDGE WASTE PILE <br />Item Y/N Comments <br />1. Quantity of waste material added to Y Approximately 1375 tans of gob were added to the <br />waste disposal area this quarter? waste disposal pile this quarter. <br />2. Waste material deposited and compacted Y <br />in 24" lifts? <br />3. Drainage alterations constructed? N No drainage alterations were constructed during the <br /> the quarter. The existing drainage pattern was <br /> maintained around the perimeter of the pile to protect <br /> outslopes and direct drainage [o ditches at the toe of <br /> the pile. <br />4. Present height of waste material? Highest height approximately 99.0 feet. A portion of th <br /> second tier of the pile was completed (except for topsoil <br /> placement) during this quarter. <br />5. New bench constructed? N <br />6. Refuse material outslope placed at Y Slopes were checked far proper placement periodically <br />1V:2H outslope? during construction. <br />7. Adequate drainage on top of completed Y Grading was maintained on top of completed fdl lifts to <br />fill? ensure proper drainage from the top of [he pile. <br />8. Bench drainage adequate? Y Bench drainage was maintained throughout the quarter. <br /> See item 3. <br />9. Outslopes covered with 4' non-toxic, Y Crest of pile partially covered with subsoil.
