The report and <br />map shall contain the following: <br />1. Location and number of acres affected during that year. <br />3 Acres <br />2. Locations and number of acres reclaimed for that year. <br />None <br />3. The location and number of acres in the various stages <br />of reclamation (backfilled, graded, topsoil replaced, <br />revegetated). <br />3 acres (sloped but not seeded) <br />4. The species of vegetation planted, the locations and <br />approximate dates of planting. <br />5. The type and approximate quantity of fertilizers, <br />organic material or soil conditioners used and <br />location of use. <br />None <br />6. An appraisal of the success of reclamation efforts, and <br />in the event of failure of any efforts, a short narrative <br />on the suspected or determined cause of such failure. <br />7. Estimates of the location and number of acres to be <br />affected in the coming year. <br />Approx. 3 acres <br />8.
