ANNUAL REPORT A. Exhibit D, Mining and Affected Land maps is included and has been modified to show the changes that occurred between January 7, 1984 & January 6, 1985, including the Technical Revision Number 1. 1) Location and number of acres affected during that year. Exhibit D shows the three areas that are presently disturbed. They are Mined Out Area (1) consisting of 73.2 acres and Present Plining Area (2) consisting of 17.775 acres and T.R. No. 1 consisting of 11 acres. Approximately 350,000 tons of sand was mined during this time period. Approximately three acres of Area(2) and one and one half acres of Area T.R. No. 1, (20 feet deep), were removed during this mining operation. 2) Locations and numbers of acres reclaimed for that year. - None. 3) The location and number of acres in the various stages of reclamation (backfilled, graded, topsoil replaced, revegetated). No new areas were reclaimed this year. 4) The species of vegetation planted, the locations and approximate dates of planting. None this year. 5) The type and approximate quantity of fertilizers, organic material, or soil conditioners used and location of use. None this year. 6) An appraisal of the success of reclamation efforts, and in the event of failue of any efforts, a short narrative on the suspected or de- termined causes of such failure. The ponding reclamation procedures now being used were changed in 1984. A pair of hydrocones was installed so that SOo of the material now being pumped into ponds was dropped out of the effluent and hauled and compacted for backfill on the slopes as required. 7) Estimates of the location and number of acres to be affected in the coming year. Approximately three acres on the north side of Area (2) will be dis- turbed during the next year. Area T.R. No.l will have minimal dis- trubance during the coming year. 8) The effective date of the information provided above shall be no more than 45 days prior to the anniversary date, This date was obtained on December 29, 1984. B. A check in the amount of $350.00 is attached.
