(1989 Supp.) and Permit No. M-80-131 <br />require you to submit $350.00 in annual fees and an annual report on or <br />before December 15 of each year, As of the date of this letter wed ave not <br />received your annual fees and report. Therefore, you are in violation of the <br />above cited statute and the provisions of your Permit No. M-80-131. Pursuant <br />to Section 34-32-124(7), C.R.S.
