You must pay them each <br />year until I'eclamaticn responsibility release is granted. The fee is for LAST <br />YEARS mining and reclamation season, and MUST be paid even if the mine was in an <br />• inactive state. It is due and payable ON OR BEFORE the permit anniversary date <br />shown below. <br />If you have requested reclamation release from the Division but your permit is not <br />terminated by your anniversary date listed below the annual fee MUST be paid. If <br />the Division does release this permit before the anniversary date please do not pay <br />this fee. <br />PLEASE NOTE: [F THE ANNUAL FEE, ANNUAL REPORT, AND I.1AP ARE NOT RECEIVED ON OR <br />BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION AND A BOARD ORDER TO PAY <br />CIVIL PENALTIES IN THE At40UNT EQUAL TO THE ANNUAL FEE OF 5225.00 b1ILL BE <br />AUTOMATICALLY ASSESSED.
