Rule 2.04.13(1): By February IS, or other such date as agreed on, each permittee shall file an
annual reclamation report covering the previous calendar year for all areas under bond. The report
shall include, but not be limited to, text, discussion and maps which address:

at the Division on February 11, 2002.
Rule 2.04.13(1)(a): The name and address of the permittee and permit number;
The report is in compliance with this rule. The name and address of the permittee and permit

Rule 2.04.13(1)(b): Location and number of acres disturbed during that year;
The report is in compliance with this rule. No land was disturbed during 2001.
Rule 2.04.13(1)(c): Location and number of acres baclrftlled and graded during that year;
The report is incompliance with this rule. No land was backfilled during 2001.
Rule 2.04.13(1)(d): Location and number acres topsoiled during that year;
The report is in compliance with this rule. No land was topsoiled during 2001.
Rule 2.04.13(1)(e): The species, location and number of acres of vegetation planted during that
year, including any augmented seeding or cultural practices; and
No land were seeded or cultivated during 2001. Weed control was performed.
