In order to maintain your mining operation in compliance with Colorado laws , the
annual fee, report, and map required by C.R.S. 34-32-116(3 and 4) and
34-32-127(2) (a)(II)(A-0) must be submitted annually ON OR BEFORE your permit
anniversary date listed below. The annual fee (this is NOT a renewal fee) , report,
and map are for mining and reclamation during the PREVIOUS year, and are due and
payable , unless your reclamation bond has been released in its entirety, by your
anniversary date.

DIVISION ON OR BEFORE YOUR ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A
BOARD ORDER TO PAY CIVIL PENALTIES IN THE AMOUNT EQUAL TO YOUR ANNUAL FEE AMOUNT OF
$175.00 WILL AUTOMATICALLY BE ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL
BE GRANTED TO ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE
(C.R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED.
If your records indicate different facts than those stated above or if you have
already forwarded your remittance , report, and map to us please inform this office
as soon as possible. Any questions concerning your annual report or fees , may be
directed to Barbara Chiappone at the Division office .
Please return the enclosed FORM with your completed MAP , and a CHECK or MONEY
ORDER for annual fees due , payable to the Mined Land Reclamation Division ,
