Phone Call Log for Bypass/Upset Reporting <br />Water Quality Control Division ~qN 3 01997 <br />Permits and Enforcement Section <br />Entity: Mountain Coal Company -West Elk Mine <br />Permit No.: CO-0038776 Person contacted: Kathy Welt <br />County: Gunnison Phone No.: (970)929-5050 <br />Start Date of Potential Upset: Mon., 20 Jan. 97 Bypass/Upset Duration: Not yet known. <br />Date & Time of Call: 22 Jan. 97, a.m. Has this happened before: Yes <br />Probable Cause: See phone report for 9:00 a.n, ., 21 Jan. 97. - <br />Written Report to Follow: Yes <br />Comments: The WOCD contacted MCC to respond to a letter from MCC dated January 21, <br />1997, received by the WQCD via facsimile on January 22, 1997. <br />The WOCD clarified that it did not approve of a bypass that would not meet effluent <br />limitations. The approval granted in March of last year was specific to that incident, and with <br />the assurance from MCC that the effluent bypassed could meet effluent limitations without <br />treatment. <br />The WQCD also advised MCC that WET testing at the upstream, downstream, and permitted <br />outfall sampling locations was only necessary every other day, rather than twice per day. <br />MCC advised the WQCD that the water emanating from the fault had a temperature of 88°F. <br />Recorded by: Kathleen Sullivan <br />Date: January 24, 1997 <br />cc- Dwain Watson, District Engineer, Field Support Section <br />David Berry/Mike Boulay, DMG <br />Original to MS-3 File <br />
