Jack Gilleland <br />C & J Gravel Products, Inc. <br />514 County Road 509 <br />Ignacio, Colorado 81137 <br />RE: C & J Gravel Products, Inc. - Permits No <br />Dear Mr. Gilleland: <br />V <br />80-83 and 80-146 <br />On October 24, 2983, the Division of Mined Land Reclamation received an <br />annual fee payment in the amount of `<100.00. To this date, we have no <br />record of having received the annual reports, as requested. Submission <br />is required by the Colorado Mined Land Reclamation Act, Article 32 of <br />Title 34, C.R.S. 1973, as amended and by the Rules and Regulations <br />promulgated thereunder. Your attention is directed to the following <br />provisions from Rule 2.4 for operators holding a regular, 112 permit and <br />Rule 3.4 for operators holding a limited impact, 110 permit. <br />2.4 Annual Reports <br />2.41 On the anniversary date of each permit year, the operator shall <br />submit a report and map (to a scale of not less than 100' to <br />the inch and not more than 660' to the inch), indicating the <br />phases of the reclamation plan which have been completed, <br />correlated with the timetable. The report and snap shall <br />contain the following information: <br />(1) Location and number of acres affected during that year, <br />(2) Locations and number of acres reclaimed for that year, <br />(3) The location and number of acres in various stages of <br />reclamation (backfilled, graded, topsoil replaced, <br />re vegetated), <br />(4) The species of vegetation planted, the locations and the <br />approximate dates of planting, <br />(5) The type and approximate quantity of fertilizers, organic <br />material or soil conditioners used and location of use, <br />(6) An appraisal of the success of reclamation efforts, and in <br />the event of failure of any efforts, a short narrative on <br />the suspected or determined causes of such failure, <br />(7) Estimates of the location and number of acres to be <br />affected in the coming year, and <br />(8) The effective date of the information provided above shall <br />be no more than 45 days prior to the anniversary date. <br />423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
