Triantos, Jr. - "Black Cat Pit" <br />File No.: M-78-304 <br />Dear Mr. Wagner: <br />On December 16, 1985, our Division received your $50.00 check for the annual <br />fee for the above captioned operation. We also received the accompanying <br />report in which you requested that the permit be transferred to your bank and <br />the operation be considered to be in temporary cessation as of April 2, 1985. <br />I have several items that I must discuss with you in relation to these <br />requests. I will try to do this in as organized and comprehensive a manner as <br />possible below. <br />In order for the permit to be transferred to your bank, a completed succession <br />of operators package must be submitted. This includes a completed succession <br />of operators form, a completed performance warranty and a completed 110 <br />application form (form alone for administrative purposes - no exhibits). <br />Copies of all of these blank forms have been enclosed with this mailing. The <br />final form needed is a completed financial warranty form which may take one of <br />several forms, the most typical being a letter of credit, a certificate of <br />deposit or a corporate surety bond. Please let me know how you wish to submit <br />the $1,200.00 financial warranty, and I will send you the proper form. <br />Once you have submitted this package and our staff has reviewed and approved <br />it, our Board can approve the permit transfer during the next regularly <br />scheduled meeting. At that point, your bank will become the operator and Mr. <br />Triantos' $1,200.00 financial warranty will be returned. Then you can <br />officially request that the mining operation be put in temporary cessation by <br />giving us the date that operations on site ceased (presumably, April 2, 1985) <br />and describing to us what actions (if any) were taken to leave the permit area <br />in an environmentally stable condition. <br />423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
