Tye Notice <br />of Intent shall contain an annual fee of 550.00 and a map or statement <br />that shall indicate: (a) the land affected during previous year; <br />(b) the land to be affected during the coming year; and (c) any land <br />reclaimed during the preceding .year." <br />• <br />Based upon our records, the anniversary date of your operation is April 21 <br />and annual fees for the year(s) 1983 , totalling $ 50.00 <br />are due on that date. <br />If your records indicate different facts, please inform this office as soon as <br />possible; otherwise, we will expect to receive the annual fees as required by <br />the statute on or before your anniversary date. Also, please include the <br />required Notice of Intent to Continue Mining Operations i<o bring the Division <br />up to date as to the progress of mining and reclamation on the site.
