Kaiser Ventures, LLC is the <br />permittee and Ms. Jan Roberts is the contact person. Her address is P.O. Box 37, Desert Center, CA <br />92239. The permit azea is 170 acres of which 50 is Federal and 120 is private located approximately <br />22 miles west of Pagosa Springs on Colorado State Highway 151, portions of Sections 29 and 30, <br />T34N, R4W. Total disturbance at the mine site totals approximately 94 acres (see attached Division <br />Summary Form). All disturbance areas have now been reclaimed. Various reclamation activities <br />have been conducted over the past 20 years on areas of the mine site as described in the permit <br />document. The Division approved a Phase III release of the AVF area in 2003 that removed 17.5 <br />acres from the permit area and 16 acres of disturbed area. <br />Limited reclamation activities including site inspections and road and channel maintenance were <br />conducted during 2004.
