annual fee, report, and map required by C.R.S. 34-32-116(3 and 4) aid
34-32-127(2)(a)(II)(A-D) must be submitted annually ON OR BEFORE yojr permit
anniversary date listed below. The annual fee (this is NOT a renewal fee) , report,
and map are for mining and reclamation during the previous year , and are due and
payable, unless your reclamation bond has been released in its entirety, by this
anniversary date.

BOARD ORDER TO PAY CIVIL PENALTIES IN THE AMOUNT EQUAL TO YOUR ANNUAL FEE AMOUNT OF
$120.00 WILL AUTOMATICALLY BE ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL
BE GRANTED TO ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE
(C.R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED.
