:~ <br />The P&A Forms (EPA 7520-14) and a copy of the Bureau of Land Management Sundry <br />Notice are enclosed for these eight wells. <br />American Soda Well 29-51 (EPA Permit Number CO3858-04652) was approved for <br />construction and drilling was commenced. This activity was ceased due to a decision <br />by the company to forgo any further development until economics for the production <br />of sodium minerals from the resource improved. A 19 inch diameter hole was drilled to <br />a depth of 250 feet before drilling was terminated. The hole was plugged back to <br />surface on June 18, 2004 and capped and marked. There is a Sundry Notice attached <br />which identifies the activity associated with the plugging back to surface of this well. <br />Enclosed are Completion Details and Sundry Notices for the following wells: <br />American Soda Well Number EPA Permit Number <br />29- 22 CO3858 -04570 <br />29- 53 CO3858 -04654 <br />29- 78 CO3858 -04657 <br />These wells have had the 4.5 inch production tubing removed and cast iron bridge <br />plugs placed in the 7 inch casing below the Dissolution Surface. These wells have been <br />placed on Long Term Stand By and will be tested to verify Mechanical Integrity and a <br />copy of the test will be submitted to the Colorado Division of Minerals and Geology. <br />As described in the attached letter dated July 28, 2004, Well 20-12 (CO3858-04552) has <br />been equipped with a Time Domain Response cable and placed into service as a <br />subsidence monitoring well. <br />Should you have any questions or require more information concerning this matter <br />please contact me at 970-285 -0407. <br />Sincerely, <br />~`C~ <br />Ed Cooley <br />Environmental Health <br />and Safety Manager <br />cc: Kent Walter/BLM <br />Elisabeth Evans/EPA <br />
