4/14/2004 Attempted to set 4lh" cement basket into short T' using, stopped at 1450. Pulled cement basket out of casing.
4/22/2004 Tripped into short string 7" casing with 5 7/8" bit to 1467 and leaned [01490'
4/23/2004 Tripped into long string 7" casing with 5 7/8" bit and leaned to 1494.
4/26/2004 Ran Gauge rigs on both T' casings, could not get past 1390'. Cleaned out [01490' on T' long string w 61/8" bit.
4/27/2004 Cleaned out T' long string with 6 7/8" mill to a reported depth of 1480', twisted off 6/18" mill and connecting sub.
4/28/2004 Cleaned out T' short string with 61/8" mill to a depth 7490'.
4/29/2004 Ran gauge rings down both T' using strings, rould not get gauge rings past 1390'. Attempted to nm 2 7/8 mment basket into
T' long string casing. Basket stopped at 1401'.
4/30/2004 Cleaned out T' long string with 61/8" mill to a reported depth of 1470'.
5/3/2004 Ran 2 7/8" basket in both short and long T' casirSgs to a reported depth of 1470'. Cemented with 5 sacks of cement on each
basket.
5/4/2001 Backed off baskets in both casings. Long string 7" casing began to Eow at surface. (Please reference undesirable event report)
5/10/2004 Pumped 10 sacks of cement on short string T' casing.
5/11/2004 Pulled 2 7/8" string from long string 7" casing and discovered [hat the cement basket remained attached.
5/12/2004 Tripped into long string 7" with 2 7/8" tubing string. Tubing stopped at 1441.
5/28/2004 Tripped in long string T' with impression block to 1441'.
6/1/2004 Tripped in long T' rasing with fishing tool assembly, mold not get past 1398'.
6/2/2004 Tripped in long T' casing with 61/8" swedge, mold not get past 1398. Tripped in with 54'd" swedge, mold not get past 1414'
6/3/2004 Performed downhole camera and caliper survey on both strings. The downhole mmem indicated that both casings were parted at
1394'.
6/17/2004 Cudd Pressure Conuol attemped to run mil tubing into 7" long string casing. The coil tubing stopped at 1391' -1394'.
7/ 6/2004 Tripped 2 7/ 8" tubing to 1440' on T' long string casing. Tripped 27/8" tubing to 1447 on T' short string using.
7/7/2061 Pumped 25 sacks of SO% Type G cement and 20% Bentonite on T' long string casing with 2 7/S" tubing at 1440'.
7/7/2004 Ran into T' short string and stopped high 4sJ 1396'.
7/7/2004 Ran into 7" long string and tagged cement ®1430'.
