F~ <br />Fi117 <br />September 270, 2005 <br />Page 2 <br />FILL PLACEMENT <br />CDMG regulations require that spoil be "placed in horizontal IiRs in a controlled manner, wncurrenuy <br />compacted as necessazy to ensure mass stability". Maacimum lift thiclmesses are not specified. As indicated <br />by previous inspection reports, lilts were placed in accordance with plans specified by CTL Thompson. <br />Spoil material was transported to the fill area using a combination of frocks and dozers. Lefi thiclmesses of <br />up to 50' were placed within the area removed from the fill face by more than 50 ft. Thinner lifts of (r-8 tt. <br />with leveling and compactive effort supplied by dozers and trucks was utilized as 5ll placement advances to <br />within fifty feet of the slope face. No fill placement occurred during this quarter. <br />CK1P/ ~l;~u IV r aL:i /\ IJ <br />As documemed by previous inspection reports, cover placemeN, seedmg and mulching occurred during the <br />second quarter 2002. During the fnst quarter 2004 topsoil was transported m the upper-most portion of the <br />fill area tying between the area seeded in 2002 and the lateral ditch placed at the upper perimeter of fi117. <br />STRUCTURAL INTEGRITY <br />The entire extxss spoil fill facility was examined for signs of failure and instability such as tension cracks, <br />ground movement, springs and seeps, and standing water. No indication of potential instability was <br />observed <br />DOCUMENTATION AND OTHER OBSERVATIONS <br />Vegetaiion density is improving across the fill area No significant erosion was noted. Established <br />drainages appear to be fimctioning as designed <br />CERTIFICATION <br />Inspection was couducbed by Ronald G. Thompson, a qualified professional and MSIIA certified inspector <br />of earth and rock-fill cerbanlrme~ and impormdments, under the direction of Mr. Charles McGlothlin, a <br />registered professional enghteer licensed in the Stare of Colorado. <br />This is to certify, to Ure best of my knowledge and belief; that Fill No. 7 construction and placement, as <br />noted since the previous certification and as determined during this inspection and by discussions with mine <br />personnel, is in accordance with designs as proposed by CTL Thompson in their report dated Apri127 a~ <br />amended July 27, 2001 and as approved by the Division of Minerals and Geology. <br />lY7ad~~ V "~-h/1~'~ <br />Ronald G.
