110(2) ANNUAL FEE LETTER PLEASE READ CAREFULLY
In order to maintain your mining operation in compliance with Colorado laws , the
annual fee, report, and map required by C.R.S. 34-32-1160 and 4) and
34-32-127(2)(a)(II)(A-D) must be submitted annually ON OR BEFORE your permit
anniversary date listed below. The annual fee (this is NOT a renewal fee) , report,
and map are for mining and reclamation during the previous year, and are due and
payable, unless your reclamation bond has been released in its entirety, by this
anniversary date.
Division records indicate you owe $120.00.

DIVISION ON OR BEFORE YOUR ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A
BOARD ORDER TO PAY CIVIL PENALTIES IN THE AMOUNT EQUAL TO YOUR ANNUAL FEE AMOUNT OF
$120.00 WILL AUTOMATICALLY BE ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL
BE GRANTED TO ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE
(C.R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED.
If your records indicate different facts than those stated above or if you have
already forwarded your remittance, report, and map to us please inform this office
as soon as possible. Any questions concerning your annual report or fees , may be
directed to Barbara Chiappone at the Division office.
Please return the enclosed FORM with your completed MAP , and a CHECK or MONEY
ORDER for annual fees due , payable to the Mined Land Reclamation Division,
1313 Sherman Street, Room 215, Denver, Colorado 80203.
