Fees are based upon a <br />schedule of E350.00 if the operation is an open mining operation an E275.00 for <br />any other operation. Also, please include t e required <br />Notice of Intent to Continue Mining Operations (Annual Report) to b ing the <br />Division up-to-date as to the progress of mining and reclamation on the site. If <br />you cannot include these materials with the fees, please incidate o the enclosed <br />sheet how soon we should expect to receive the material. <br />Submittal of a report and map (to a scale of not less than 100' to the inch and <br />not more than 660' to the inch), indicating the phases of the recla ation plan <br />which have been completed and correlated with the timetable is required.
