• SECTION 3 -CDMG ARR FORM AND SUPPORT DOCUMENTS <br />RULE REQUIREMENT <br />RiJLE 2.04.13(1) (a-t) <br />2.04.13(1) by February 15, or other such date as agreed on, each permittee shall <br />file an annual reclamation report covering the previous calendar year for all areas <br />under bond. The report shall include, but not limited to, text, discussion and maps <br />which address: <br />o (a) the name and address of the permittee and permit number <br />o (b) location and number of acres disturbed during that yeaz <br />o (c) location and number of acres backfilled and graded during that year <br />o (d) location and number of acres backfilled and graded during that yeaz <br />o (e) the species, location and number of acres of vegetation planted during <br />that yeaz, including any augmented seeding or cultural practices <br />o (f) location, number of acres and date of planting for all previously <br />revegetated areas <br />• GENERAL DISCUSSION <br />CDMG Report -During 2003, 120 acres were disturbed, 7.2 acres were backfilled and <br />graded and 7.2 acres were topsoiled. Colowyo permanently seeded or planted 69.8 acres <br />in 2003. Through 2002, 1397.98 acres have been reclaimed at Colwoyo. A total of <br />679.79 re-seeded acres from 1978 through 1993 have been in existence for greater than <br />10 yeazs. 472.72 acres within the permit area aze in long-term mining or reclamation <br />facilities and the active mining area is comprised of 1784.47 acres (See Exhibit 3A for <br />Iocation ofreport}. <br />
