Enclosed with this cover letter summarizing site <br />activities during the year ending September 29, 2006, are: (i) a signed DMG <br />"ANNUAL FEE and REPORT REQUEST" form; (2) a check from RMA for <br />$688.00 for the annual fee due on or before September 29, 2006; (3} a map <br />depicting general site conditions for the reporting year; and (4) an air photograph <br />reproduction at the same approximate scale as the map showing .Tune 2006 <br />conditions for your reference. We have included both the map and air photo in <br />11"x17" fonnaY for convenience. We would be happy to also provide them in <br />22"x34" format if desired. <br />During the report yeaz mining activity occurred within Phases 8 and 9 as per <br />Exhibit D-1, Revised Mining Plan Map in Technical Revision TR-002 of the <br />permit, and as shown on the attached Annual Report Map. Other site activities <br />and improvements included (I) stripping and stockpiling of topsoil and <br />overburden from within Phase 9 and within a small portion of Phase 1 l; (2) initial <br />reclamation backfilling of the west and south sides of Cell No. 2 (Phases 7 and 8 <br />from west to east); and processing and removal of old recycled concrete <br />stockpiles formerly located in the northern portion of Phase 9. Phase 3B remains <br />an active fresh/sedimentation pond area. <br />Mining activities at the site occurred within [he two phase areas noted above. <br />z3z9 w Ma;n screec Disturbance consisted of the approximately 28.7 acres where mining was <br />sae zol expanded compared to the prior reporting year.
