As required by CRS 34-32.5-116, we are providing this Annual Report for the Heit Sand and
Gravel Mine, DMG file M-2003-016.
In September 2004, Civil Resources, LLC on behalf of SW Chambers, LLC, applied to the DMG
for Technical Revision No. 1 (TR) to allow a change in phasing. The TR requested Phase I
include installation of 10 monitoring wells, installation of the access road to scale house (860' x
25'), removal of topsoil from processing area (2.8 acres) and begin installation of a temporary
topsoil berm. Phase II include mining operation and reclamation.
In September 2005, Wright Water Engineers, Inc. on behalf of SW Chambers, LLC, applied to
the DMG for Technical Revision No. 2 to revise the groundwater trigger, revise the language on
the process to be followed for implementation of mitigation measures and change the monitoring
frequency of monitoring of the piezometers to bimonthly beginning in October 2005 with
monthly monitoring resuming upon initiation of mining activity.
^ The extent of current disturbances:
During the last permit year (September 2004-September 2005), the 10 monitoring wells were
installed and the monitoring program begun. Attached please find a plan showing the well
locations and data collected. This information was previously submitted to the Division on
September 14, 2005 with TR No. 2.
To date no topsoil removal or access road construction has been done at the site.
^ Reclamation accomplished to date and during the preceding year:
During the last permit year (September 2004-September 2005) no reclamation has been
accomplished at the site.
