Please be advised that according to records of the Division of Minerals and
Geology the following annual fee(s) and report(s) are past due. The )ivision
MUST receive ALL fees due no later than 4 p.m. on April 16, 1993, or a Notice
of Violation will be issued for failure to pay annual fees (for each unpaid
annual fee) and submit an annual report on the permit anniversary data
(C.R.S. 34-32-124(1)). Also, civil penalties will be automatically assessed
equal to the fees due IC.R.S. 34-32-124(7)).
Please be aware that no permits for new mining operations shall be gr,inted to
any operator who is currently found to be in violation in this state
(C.R.S. 34-32-120).
