Fees are based <br /> upon a schedule of $350.00 if the operation is an open mining operation and <br /> $275.00 for any other operation. Division records indicate you owe $350. <br /> Permit No. : M-77-184- <br /> Operation Name: FT COLLINS PIT <br /> Anniversary Date: 10 / 30 / 85 <br /> Annual Fees for the Year(s) : 1985 <br /> Total : $350 (Due on the Anniversary Date) <br /> If your records indicate different facts, please inform this office as soon as <br /> possible, otherwise, we will expect to receive the annual fees as required by <br /> the statute on or before your anniversary date. Also, please include the <br /> required Notice of Intent to Continue Mining Operations (Annual Report) to <br /> bring the Division up-to-date as to the progress of mining and reclamation on <br /> the site. If you cannot include these materials with the fees, please <br /> incidate on the enclosed sheet how soon we should expect to receive the <br /> material . <br /> Submittal of a report and map (to a scale of not less than 100' to the inch <br /> and not more than 660' to the inch), indicating the phases of the reclamation <br /> plan which have been completed and correlated with the timetable is required. <br /> The report and map shall contain the following information: <br /> 1 ) Location and number of acres affected during that year, <br /> 2) Locations and number of acres reclaimed for that year, <br /> 3) The location and number of acres in the various stages of <br /> reclamation (backfilled, graded, topsoil replaced, revegetated) , <br /> 4) The species of vegetation planted, the locations and approximate <br /> dates of planting, <br /> • <br /> 423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
