LOng <br />Division Director <br />On May 7, 1998, the Division received an incomplete application package for the Van 4 Shaft <br />Operation. On May 9, 1998, the Division received a copy of a certified mail return receipt card <br />which, according to the cover letter, was for the proof of providing notice of the application to <br />the San Miguel Basin Soil Conservation District. However, the copy of the return receipt card <br />received by the Division showed the wrong side of the return receipt card, and it was not possible <br />to verify that the addressee on the card was the Soil Conservation District. On May 20, 1997, the <br />Division sent International Uranium Corp. (then Energy Fuels Nuclear, Inc.) a letter (copy <br />attached) listing application exhibits that were missing and explaining that those items must be <br />received before the Division could consider the application as being submitted and technical <br />review of the application could begin. In addition, the letter indicated that the wrong type of <br />application form had been used and as such the applicant needed to prepare a new application <br />using the appropriate type of application form, a copy of which was attached to the letter sent to <br />the applicant by the Division. To date. the Division has not received any of the missing <br />~lication materials nor has the Division received an appropriately com lep ted application form. <br />Also, since that time the name of the company has changed from Energy Fuels Nuclear, Inc. to <br />International Uranium Corporation and the applicant and permitting contact addresses and phone <br />numbers shown on the original application appear to have changed as well. <br />In order to continue the application process, the applicant needs to provide all of the missing . <br />information listed in the Division's May 20, 1997 letter, correct the application form to show the <br />current company name, contact persons, addresses and telephone numbers, properly initial the <br />appropriate portions of the application form, complete the signature/notarization portions of the <br />application form, and provide a copy of the correct side of the certified mail return receipt card <br />from the San Miguel Basin Soil Conservation District. The Division reiterates that this <br />information must be received before the Division can consider vour application as beine submitted <br />and technical review can beein.
