A summary of <br />the report and requirements of the submittal in accordance with Rule 2.04.13(1) follows: <br />a) Narne and address of perntittee and permit number. The petmittee and operator during <br />1996 was; <br />Basin Resources, Inc. <br />14300 Highway 12 <br />Weston, CO 81091 <br />Permit No. C-81-012 <br />b) Location and number of acres disturbed during that year. No new disturbances were <br />initiated during 1996 <br />c) Location and number of acres backfilled during that year. The 1996 Annual Reclamation <br />Report states that the Apache Canyon air shafts no. 1 and 2 were reclaimed in 1996. <br />Does the Division have verification of how the shafts were sealed? The report simply <br />states that concrete foundations were demolished and transported to the Golden Eagle <br />concrete disposal area, then this area was ripped and graded. Do we have verification <br />that the shaft was sealed in accordance with the appropriate rules and permit plan? Basin <br />Resources needs to elaborate on the shaft sealing for purposes of assuring that the shafts <br />were sealed appropriately when bond release is requested. <br />d) Location and number acres topsoiled during that year. The report indicates [hat topsoil <br />was redistributed at Apache Canyon airshaft No. 2, 0.3 acres 3-6" thick. <br />e) The species, location and number of acres of vegetation planted during that year, <br />including any augmented seeding or cultural practices. The report indicates that no <br />seeding occurred during "1997". The Division believes this to be a typo and should read <br />1996. <br />f) Location, number of acres and date of planting for all previously revegetated areas. <br />Basin has included a table defining the locations and years of previous reclamation <br />activity. This totals 5.7 acres. <br />STATE OF COLORADO <br />g) The report indicates that reclamation maps would be submitted. Were these ever <br />provided:" <br />
