Dept. of Natural Resource <br /> Based upon our records, the anniversary date of your mining operation permit is <br /> October 30 and annual fees for the year(s) 1982 totalling <br /> $ 350.00 are due on that date. Fees are based upon a schedule of <br /> $350.00 if the operation is an open mining operation of 10 acres or more and <br /> $275.00 for any other operation 10 acres or more. <br /> • Also, to bring the Division up to date as to the mining and reclamation on the <br /> site, please submit a report and map (to a scale of not less than 100' to the <br /> inch and not more than 660' to the inch) , indicating the phases of the reclama- <br /> tion plan which have been completed, correlated with the timetable. The report <br /> and map shall contain the following information: <br /> 1) Location and number of acres affected during that year, <br /> 2) Locations and number of acres reclaimed for that year, <br /> 3) The location and number of acres in the various stages of reclamation <br /> (backfilled, graded, topsoil replaced, revegetated) , <br /> 4) The species of vegetation planted, the locations and approximate dates <br /> of planting, <br /> 5) The type and approximate quantity of fertilizers, organic material or <br /> soil conditioners used and location of use, <br /> 6) An appraisal of the success of reclamation efforts, and in the event <br /> of failure of any efforts, a short narrative on the suspected or determined <br /> causes of such failure, <br /> 7) Estimates of the location and number of acres to be affected in the <br /> coming year, and <br /> 8) The effective date of the information provided above shall be no more <br /> than 45 days prior to the anniversary date. <br /> • <br />
