Thz report shall also provide <br />proof that thzre are no past-due property taxes. <br />(i) Per the requirements of 3 CCR -i07-I, d07-q, and Rules d.7(3), prnniU secured with an <br />Irrevocable Letter of Credit, the operator shall havz the hank providz focumentation in the form <br />of a balance sheet, czttrtied by a Certified Public Aee0untant, demonstrating that the Letter of <br />Credit doe; no[ zxceed ten percent (10°c) of the bank's capital surplus accounts. <br />(ii) Per the reyuirements of 3 CCR 407-], -307-4, and Rules 4.9.3(c), permits secured by a First <br />Priority Lien on any fixurres and Equipnten[. the operator shall provide a report in sufficient <br />detail to fully describe the condition, value, and location of all pledgzd fixtures and equipmznt. <br />(h) Per the requirements of 3 CCR -107-I. 407-a, and Rulz~ I.I S(I)(c), all menitorin~ information <br />required as pan of [he approved penmt application, to include the Environmental Protection Plan. (Please <br />notz that if you are not a Desi~~nated h4inins Operation, you will not ha+e an Environmental Protection <br />Plan as part of your approved pzrmit.l <br />
