Long, <br />Division Director <br />This letter is to inform you that your company is the apparent low bidder on <br />Project RN -MINES -292, the Poverty Gulch /Tenderfoot Project. To complete the <br />process of bid award, you must send the following information to me within ten <br />(10) calendar days from receipt of this notice: <br />1) A certificate of insurance in the amounts given in Number 5 of the <br />General Conditions, and the additional amounts stipulated in the <br />three -way "Agreement for Reclamation Activities ". The certificate <br />of insurance must show the State of Colorado as an additional <br />insured. You must also supply an insurance certificate for: <br />Mr. Edgar T. Hunter, Pikes Peak Mining Company, P.O. Box 191, <br />Victor, Colorado 80860. The certificate must name Pikes Peak <br />Mining Company as an additional insured, must include a waiver of <br />subrogation for Pikes Peak Mining Company, and provide for 30 days <br />notification of cancellation. <br />2) A project schedule, as specifies in Number 7 of the General <br />Conditions. <br />3) A performance bond and a labor and materials payment bond in the <br />amounts of 100 percent and 100 percent respectively of the bid <br />amount (562,364.22). The bonds must be submitted using the State <br />forms. Copies are enclosed. <br />4) All signed copies of the enclosed three -way ".Agreement for <br />Reclamation Activities ". <br />S TAR OF COLORADO <br />When the above documents have been submitted and found to be in ordei, a <br />purchase order will be issued and a notice to proceed will be given. <br />
