Fees are based <br /> upon a schedule of $350.00 if the operation is an open mining operation and <br /> $275.00 for any other operation. Division records indicate you o$950. <br /> Permit No. : M-74-052 <br /> Operation Name: Pit No 1 <br /> Anniversary Date: 06/26/87 <br /> Annual Fees for the Year(s) :1987 <br /> Total : $350 (Due on the Anniversary Date) <br /> If your records indicate different facts, please inform this office as soon as <br /> possible, otherwise, we will expect to receive the annual fees as required by <br /> the statute on or before your anniversary date. Also, please include the <br /> required Notice of Intent to Continue Mining Operations (Annual Report) to <br /> bring the Division up-to-date as to the progress of mining and reclamation on <br /> the site.
