You must pay <br />these fees each year until reclamation responsibility release ie granted. The fee <br />listed below is for LAST YEARS mining and reclamation season, and RUST be paid even <br />if your operation was in an inactive state. The fee is due ON OR BEFORE the permit <br />anniversary date shown below. <br />If you have requested reclamation responsibility release from the Division but your <br />permit is not released by your anniversary date listed below, the annual fee RUST be <br />paid. If the permit ie released before the anniversary date, please do not pay the <br />annual fee. <br />PLEASE NOTE: IF THE ANNUAL FEE, ANNUAL REPORT, AND MAP ARE NOT RECEIVED A NOTICE OF <br />FORMAL PUBLIC HEARING BEFORE THE MINED LAND RECLAMATION BOARD FOR POSSIBLE PERMIT <br />VIOLATIONS, PERlSIT REVOCATION, AND POSSIBLE CIVIL PENALTIES WILL BE ISSUED.
