Dear Messrs. Matthias and Hill:
Based upon our records, t_1e anniversa\dateof your mining operation permit is
April 23 and annual fees for the year(s) 81-82 , totalling
$ 350.00 are due on that date. Fees are b sed upon a schedule of
$350.00 if the'operation is an open ruining operation oaf 10 acres or more and
$275.00 for any other operation 10 acres or more.
Also, to bring the Division up to date as to the mining nd reclamation on the
site, please submit a report and map (to a scale ofynot 1 ss than 100' to the
inch and not more than 660' to the inch) , indicating the oases of the reclama-
'ion plan which have been completed, correlated o-iith the timetable. The report
and man shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed for that year,
3) The location and number of acres in the various stages of reclamation
(backfilled, graded, topsoil replaced, revege�ated) ,
4) The species of vegetation planted, the locations and approximate dates
of planting,
5) The type and approximate quantity of fertilizers, orcan_c material or
soil cond-tioners used and location of use,
6) An appraisal of the success of reclamation efforts, and in the event
of failure of any efforts, a short narrative on the suspected or determined
causes of such failure,
7) Estimates of the location and numoer of acres to be affected in the
coming year, and
8) The effective date of the information provided above shall ba no ;,.ore �
']� than 45 days prior to the anniversary date.
