Grandview Dr. <br />Tempe, AZ 85281 <br />Annual Report: Deep Creek Placer (Permit No. M-1986-102) <br />Sept. 4, 2004 <br />Deaz Sir or Madam: Please accept this Annual Report on the "Deep Creek Placer," a <br />temporarily inactive limestone mine 7 miles west of Telluride, Colorado at the confluence <br />of Deep Creek and the San Miguel River. <br />--No mining has taken place intheyeaz Sept. 5, 2003 =Sept. 4-2004- - - --- <br />--Portals to the mine have been checked and they aze secure. <br />--A new heavy duty lock and hasp has been installed on the "equipment room" entrance. <br />--Routine maintenance of access road has been carried out. <br />--Annual maintenance of stormwater "ponds" along access road for capturing runoff has <br />been carried out. <br />--Access road gate is secure and locked. <br />--A letter dated July 13, 2000, discussing the Existing Bond on the Mine, was received <br />from Bob Oswald, Environmental Protection Specialist in the Durango Office. It was <br />mailed to the current pennittee contact, Michael Moore of B-Mining, PO Box 326, Nucla, <br />Colorado 814241ast year and copied to myself and Suzi Ericksen. The letter indicated <br />that the existing bond is sufficent ($12,000). <br />--See enclosed map showing existing permitted area. <br />--No disturbances to the surrounding ]and or the mine itself are anticipated in the coming <br />yeaz. <br />--Regular maintenance of roads, equipment apron, portals, and stormwater ponds will be <br />conducted by myself over the course of the coming yeaz. <br />The Mine is part of a property owned by the Collins Living Trust. I am a co-trustee of the <br />Trust-along with my-two sisters, Jean Collins-Norris (Chatham, Massachusetts) and Lois <br />Morehead (Chico, California). My mother, Kathryn O. Collins, is still alive and living in <br />Tempe, Arizona. <br />The current permittee for the mine (Michael Moore of Nucla, Colorado) has not actively <br />removed material in the past yeaz. However, there are on-going discussions in the family <br />and with the permittee that may lead to a bid to reopen the mine in the future.
