Box 983 Fort Morgan, Colorado 80701 Re: George Westhoff Pit, Permit No. 77-94 Dear Mr. Westhoff: A review of our file indicates the anniversary date of this permit (which corresponds to the permit issuance date) is December 20th. In the past, the Division has attempted to contact you regarding the necessity of submitting an annual report and annual fees on the anniversary date. Unfortunately, as you can see on the two enclosed correspondences, the Division sent these notices to the wrong address. The Division iias corrected this wrong address error. As of December 20, 1983, Morgan Sand and Gravel Company has not suomitted any annual report or annual fees. The operator needs to submit an annual report and map pursuant to P,ul2 2.41 of the Rules and Regulations: 1) Location and number of acres affected during that year, 2) Locations and number of acres reclaimed for that year, 3) The location and number of acres in the various stages of reclamation (backfilled, graded, topsoil replaced re vegetated), 4) The species of vegetation planted, the locations and approximate dates of planting, 5) The type and approximate quantity of fertilizers, organic material or soil conditioners used and location of use, 6) An appraisal of the success of reclamation efforts, and in the event of failure of any efforts, a short narrative on the suspected or determined causes of such failure, 7) Estimates of the location and number of acres to be affected in the coini ny year, and 8) The effective date of the information provided above shall oe no more than 45 days prior to the anniversary date. ~~ ~ti~ 423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
