Colorado 80203 <br />Re: Permit No. M-1990-111, Annual Report <br />Dear Mr. Keffelew: <br />Cripple Creek & Victor Gold Mining Company ("CC&V") is submitting the required <br />annual report, payment (check # 00047730), and map for the Moon Anchor Dump, <br />Permit No. M-1990-11 I .The financial warranty for this permit is included in the <br />Amendment No. 8 financial warranty as recommended by the Division and as approved <br />by the Mined Land Reclamation Board on August 23, 2000. Per that approval, CC&V <br />has formally requested release of [he individual financial warranty for this permit. As <br />such, this will be the last annual report provided for Permit No. M-1990-1 11. <br />Reportable Conditions: <br />Per the requirements of C.R.S 34-32-116(3), and C.R.S. 34-32.5-116(3)(a): <br />Reclamation accomplished to dare: 9.0 acres have been graded, stabilized, seeded, and <br />fertilized. <br />Reclamation nccanplished during the precedent; year: None. <br />New disturbances anticipated during the upcarring year: None. <br />Reclntnation rrnlicipared during the upcoming year: None. <br />
