the permit, a Notice of Intent to Continue MininD Operations. The
Notice of intent shall contain an annual fee of 550.00 and a map or
statement that shall indicate: ia) the land affected during the
previous year; !b} the land to be affected during the coming year;
and (c) any land reclaimed during the preceding year." Qivision
