Box 928 <br />Meeker, CO. 81641 <br />Subject: Submittal of Undesirable Event Report <br />Dear Mr. Mehlhoff'. <br />Enclosed please find Rf;port of Undesirable Event regarding hose rupture <br />October 11, 1995. The hose was utilized to direct water from cavity #1 to waste <br />pond for disposal throu;;h surface plastic pipes. Shortly after start-up Steve <br />Schram noticed a pressuue drop in the control room and Bob Warneke noticed <br />pressure loss at waste pond. In one to two minutes the system was shut down. <br />The new concrete pad contained the majority of the spill and directed the water <br />to the tank farm. The water spilled contained high sodium chloride (~17%) and <br />sodium bicarbonate (~2'%). Approximately 200 gallons of water was sprayed <br />outside the pad and wa:; limited to the disturbed plant area and approximately <br />2/100 of an acre in size. After the soil dries, the surface contaminated material <br />will be disposed in the approved lined pit.
