In order to maintain your mining operation in compliance with Colorado laws, the
annual fee, report, and map required by C.R.S. 34-32-116(3 and 4) and
34-32-127(2)(a)(II)(A-D) rt~ust be submitted annually on or before your permit
anniversary date. The annual fee (this is NOT a renewal fee), report, and map are
for the 1991-92 mining and reclamation season, and are due and payable, unless your
reclamation bond has been released in its entirety, before this anniversary date.

money order for your annual fee, payable to the Mined Land Reclamation Division,
1313 Sherman Street, Room 215, Denver, Colorado 80203. The Division cannot accept
foreign currancy or checks drawn on foreign banks.
